Nursing Care Plans for NDIS Participants

Flame Lily creates practical, audit-ready Nursing Care Plans (NCP) for NDIS participants with complex needs. These plans are commonly required where supports fall under NDIS High Intensity Daily Personal Activities and other higher-risk routines. Plans are written for real-world delivery, with clear, step-by-step guidance that is easy for support teams to follow across shifts.

  • Care plans for High Intensity Daily Personal Activities and complex supports
  • Telehealth Australia-wide (face-to-face video consults via Teams/Zoom)
  •  In-person across Wollongong, Shoalhaven/Nowra, Southern Highlands, Campbelltown, Western Sydney and surrounding suburbs
  • Travel available for bulk care plans for provider teams
  • Optional training, competency check-offs and scheduled reviews

Audit-ready Nursing Care Plans are essential for providers supporting NDIS High Intensity Daily Personal Activities, where clinical governance and documentation are closely assessed during audits.

Audits typically look for:

  • The plan: RN-written/reviewed and current
  • The training: staff competency aligned to the participant’s plan
  • The evidence: notes/logs showing the plan is followed

For providers, support coordinators, participants and families

If a participant has high-risk supports (e.g., PEG feeding, catheter care, complex wounds, epilepsy/seizure management), teams need more than verbal instructions. A structured Nursing Care Plan helps ensure care is delivered consistently, safely, and in line with clinical governance expectations.

These plans are commonly requested by:

  • NDIS support coordinators and provider teams managing complex/high-risk supports
  • Disability support providers (SIL, in-home support, community access)
  • Families supporting someone after hospital discharge or during a change in needs
  • Aged care/Home Care Package providers seeking specialist nursing planning (via registered providers)

Bulk Care Plans for Provider Teams

We can support providers needing multiple care plans across participants, ideal for new intakes, audit preparation, or post-hospital transitions. Bulk care plan projects can be delivered face-to-face Australia-wide via Teams/Zoom, with in-person delivery available where needed and feasible.

What’s Included in a Nursing Care Plan

Each plan is tailored to the individual and may include:

  • Participant overview and goals
  • Clinical risks, red flags, and escalation steps
  • Required equipment and supplies
  • Step-by-step care instructions (in plain language)
  • Infection control and safety precautions
  • Documentation tools (e.g., logs/charts where relevant)
  • Clear guidance for support workers, families and care teams
  • Review recommendations and update triggers (e.g., condition change)

If training is required, we can align it with the individual’s plan and support its implementation.

Training, Competency and Regular Reviews

Some teams don’t need a permanent nurse but they do need the right oversight around high-risk supports. We can help by:

  • Training support workers against the participant’s plan
  • Competency check-offs (where appropriate and agreed)
  • Scheduled reviews (weekly/fortnightly/monthly depending on needs and funding)
  • Post-hospital check-ins and wound progress support
  • Updating plans as conditions or routines change

Need ongoing oversight? See Nursing Support Services (RN Oversight).

NDIS Nursing Care Plans FAQs

  • What is a Nursing Care Plan (NCP)?

A Nursing Care Plan (NCP) is a structured document that guides safe day-to-day support for complex needs. It clarifies routines, risks, escalation steps and documentation requirements so care can be delivered consistently across the support team.

  • Who writes the care plans?

Plans are written by an experienced Registered Nurse and tailored to the participant’s needs, goals and support environment.

  • What makes a care plan audit-ready?

An audit-ready Nursing Care Plan (NCP) is clear, structured and specific to the participant. It includes step-by-step instructions for safe delivery, identifies risks and control measures, and sets out escalation guidance, so support teams know what to monitor and when to act. It also documents training/handover expectations and review triggers (e.g., changes in health status, routines, equipment or incidents) to support clinical governance.

  • What information do you need to create a Nursing Care Plan?

A short intake summary is usually enough to start: the participant’s goals, current routines, key risks, equipment used, support team set-up, and any relevant reports or discharge summaries (if available). We’ll confirm exactly what’s needed during intake.

  • What types of Nursing Care Plans do you create?

Plans are tailored to the participant’s needs and may include care plans for:

    • PEG/enteral feeding
    • Mealtime management & dysphagia (where required)
    • Catheter care (including SPC/IDC routines)
    • Complex bowel care
    • Continence care planning (where appropriate)
    • Stoma/colostomy care
    • Wound care (including post-hospital wounds)
    • Pressure care / skin integrity
    • Epilepsy / seizure management
    • Diabetes supports & subcutaneous injections (where appropriate)
    • Respiratory supports / suction routines (where clinically appropriate)
    • Tracheostomy care
    • Medication supports for complex routines (within scope and with appropriate safeguards)

  • Can you do this via Telehealth?

Yes. We offer face-to-face video consults (Teams/Zoom) Australia-wide. In-person appointments are available across select NSW areas (including Wollongong/Shellharbour/Kiama, Shoalhaven/Nowra, Southern Highlands, Campbelltown and Western Sydney, plus surrounding suburbs).

  • How long does it take to complete a care plan?

Timeframes depend on complexity and how quickly key information is provided. We’ll confirm scope and expected turnaround after intake.

  • Do you train support workers on the plan?

Yes. Training can be delivered in person or via Teams/Zoom and can be matched directly to the participant’s plan so staff understand the routines, risk controls and escalation steps.

  • Can you provide competency support for support workers?

Where appropriate and agreed, we can support competency development through structured training, guidance and review processes aligned to the participant’s plan and provider requirements.

  • How often should care plans be reviewed?

Review timing depends on complexity and risk. We can recommend review triggers and provide scheduled check-ins where needed (e.g., weekly/fortnightly/monthly depending on needs and funding).

  • Do you provide ongoing RN oversight after the plan is written?

Yes. If required, we can provide Nursing Support Services (RN oversight) to help teams implement the plan safely through coaching, supervision and structured reviews.

  • Do you work with support coordinators and providers?

Yes. With consent, we can liaise with support coordinators and providers to ensure the plan is implemented clearly and consistently across the support team.

  • Can you help after hospital discharge?

Yes. We often support teams after discharge—especially where wounds are fresh, routines are new, or equipment has recently changed.

  • Do you guarantee NDIS funding approval?

We provide thorough assessments, documentation and recommendations, but funding decisions are made through the NDIS and the participant’s plan.
