Nurse Support Delivered At Home

Flame Lily provides community nursing for NDIS participants, and may also support Home Care Package clients via registered providers where appropriate. Our nurses deliver respectful, person-centred care that supports wellbeing at home, whether needs are short-term (such as after hospital discharge) or ongoing. We support NDIS participants, families and provider teams with day-to-day health routines, monitoring and clinical tasks, helping ensure care is delivered safely at home.

Vital signs checks, general health monitoring and early escalation support where required.

Epilepsy / Seizure Support

Monitoring and response guidance aligned to an individualised seizure plan.

Pain Management Support

Support with managing chronic pain or discomfort related to the participant's disability, including monitoring, comfort routines and escalation guidance.

Nursing support and observation for participants requiring assistance with ventilator equipment and respiratory routines at home.

For higher-risk routines or teams needing structured governance, we also offer Nursing Care Plans (NCPs) and/or Nursing Support Services (RN Oversight).

When Community Nursing is Helpful

Community nursing is often used when participants or teams need support with:

  • Post-hospital transitions and settling into new routines
  • Ongoing wound care routines and monitoring
  • Catheter, bowel, stoma or continence supports
  • Diabetes routines and injections (where clinically appropriate)
  • Medication routines and documentation support (within scope)
  • Health monitoring and early escalation to reduce risk

In-Home Respite Nursing

We offer in-home respite nursing to give carers short-term support while maintaining safe, consistent nursing routines at home. This service can help during periods of increased care needs, after hospital discharge, or when carers require a planned time out.

Availability depends on location and clinical requirements.

Funding Pathways (NDIS & Home Care Packages)

NDIS

Community nursing may be funded where supports are disability-related and considered reasonable and necessary under the participant’s plan.

Home Care Packages

Services are delivered through registered providers. Flame Lily may be engaged as a subcontracted nursing provider where appropriate.

    Support may include wound routines, continence supports, catheter/stoma/bowel routines, monitoring, diabetes routines and injections (where appropriate), and practical guidance for safe delivery and escalation.

    Yes. We often support post-hospital transitions—especially where routines, wounds, equipment or risk levels have recently changed.
